For the first time, three generations of a single family officiated the same basketball game, marking a significant milestone in a family tradition spanning over 40 years. Bill Cain Jr., the patriarch, estimates he has refereed nearly 1,000 games across various levels, from youth leagues to college.
However, this recent game holds a special place, distinct from all the others in his four-decade-plus career.
When Cain Jr. looked at the officiating schedule, he realized that for the first time in his long career, he would be working a game alongside his son, Bill Cain III, and his grandson, Bill Cain IV, creating a truly memorable family moment on the court.
“When I saw on the schedule that we were all refereeing together, it just melted my heart,” Cain Jr. told WBNS.

The same court saw three generations of the same family officiating, all wearing the referee stripes. The grandfather, Cain Jr., contributed decades of experience; his son offered confidence and command; and his grandson brought energy and enthusiasm.
The Cain family has a long and celebrated history in sports officiating. Bill Cain Jr., Bill Cain III, and Bill Cain IV continued this legacy, gaining the necessary expertise not through formal, structured instruction but instead through valuable, practical experience on the court together. This family tradition was their main training method.
“He kind of taught me everything I know, and it came full circle because now, Billy is the same way,” Cain III recounted.
“He started a long time ago. Awesome that it’s gotten to me. I’m really honored that he brought both of us into it,” Cain IV said.
The Cain family’s tradition is officiating, a commitment passed down through the sound of the whistle and a strong adherence to fairness and consistency in sports.
They are highly respected referees across various sports, recognized with many awards, and have officiated numerous significant, high-profile games throughout their careers.
Stepping onto the court together for the first time, however, meant more than all their previous achievements.
Cain Jr. noted, “It was the most important game I ever did. That one touched my heart.”
For the Cains, officiating is more than just a job; it’s how they stay connected to the sport, the athletes, and each other—a tradition that shows no signs of slowing down.
Cain Jr. smiled when contemplating whether the family’s referee legacy would continue. “Who knows,” he remarked, suggesting the possibility of a “Bill Cain V.”
